Ask for a referral

Contact us if you want to move into a care home or are looking for a care home for a family member.  

You can:  

  • refer yourself 
  • ask a family member, friend or carer to refer you 
  • get a referral from your doctor, nurse or other health and social care professional

When you contact us, we’ll work with you to understand your strengths by doing an assessment.  

An assessment is not a test, it’s a way for us to meet, listen to you and work out what support can help you meet your goals.  

You can bring a family member, friend or carer to these meetings.  

In the assessment, we’ll discuss: 

  • ways you can continue to live in your home such as reablement or home care 
  • how we can increase any existing home care to meet any changes in your needs 
  • other care options including nursing or residential care  

You will have a full say and involvement in your assessment. If you’d like someone to speak up for you and what you want, you can also bring an advocate.  

If we need help in the assessment, we may speak to other people involved in your care. This may be a doctor or a support worker.
