Assessing children who have disabilities

We'll complete a Child and Family Assessment if your child has a:

  • disability
  • serious health condition
  • sensory impairment

The assessment will include how your child's disability impacts on their:

  • daily life
  • wishes and feelings
  • family

The assessment might recommend:

  • support from one or more of our services
  • support from other organisations, such as the NHS
  • short breaks

Most children with disabilities have their needs met through targeted short breaks.

However, if your family needs additional support, we'll include this in a Disabled Child Support Plan. The plan will focus on developing your child's independence. We'll develop the plan with you and your family, exploring the different support options available.

You may be offered a personal budget to arrange the care you need.
