Details about you and your household 

We need: 

• personal details, nationality, health and support needs for yourself and all members of your household that need to move 
• details of your current housing situation and why you need help  

Passport or other form of ID 

We need this for every member of your household that needs to leave with you. A passport is best, if you have one. 

If any family member that needs our help is not a British citizen, we need proof of their immigration status.  

Proof of where you live 

This could be a tenancy agreement or a utility bill, for example.
