How we support care leavers aged 16 to 25
You can get a range of support from:
- a personal adviser
- a specialist worker from the Virtual School
- a single point of contact (SPOC) at the job centre
- specialist mental health workers
They can also point you to relevant support and services for young people.

If you're leaving care or have already left care, we still want to make sure that you feel safe and supported, and know where and who to go to for help.

If you’re aged 16 to 18
If you’re a young person in our care aged 16 to 18, your allocated social worker will continue to provide the key care-planning role until you reach the age of 18.
You’ll also have an allocated personal adviser who will work alongside your social worker and support you with the transition from care.
You’re also entitled to:
- a needs assessment
- a pathway plan – this helps you with your current plans, and plans for leaving care
If you’re aged 18 to 25
If you’re aged 18 to 25, you’re entitled to:
- stay in touch
- review your pathway plan
- support from a personal adviser
- support to help you with your aspirations including employment, education and training
- assistance in general
Care leavers’ forum
If you’re a care leaver, you can join our care leavers’ forum to share your insights on the services you’ve received and improve it for other children in our care.
