If you go into hospital or a rehabilitation centre, it’s good to plan what you’ll need to go home.  

This may be: 

  • how you’ll get home 
  • making sure you have food for when you get back 
  • restarting any care you were getting before. To do this tell us, private care providers or personal carers your hospital discharge date 

How to ask for support to return home 

Before you leave hospital, they will ask you questions to check it’s safe for you to go home. 

If you need support before going home, speak to someone who is caring for you.  

They can make a referral so you can get assessed to see what support you may need.   

You may be eligible for short term care to help you rebuild your confidence to live independently.  

Short term care could be:  

  • Intermediate care - rehabilitation unit where you can get hands on care available 24 hours a day 
  • Reablement care – where you get support in your home whilst regaining independence 

This may be free for up to 6 weeks. 

If you need care after this, we will work with you to see how much you’ll need to pay towards it.
