What you need to apply
Send copies of these documents with your application form.
Proof of your name and age
We need one of these. It can be a photocopy of your:
- valid passport
- biometric residence permit - a copy of both sides
- valid driving licence
- birth or adoption certificate if your name has not changed
- marriage certificate
- European ID card
Proof of address
We need one of these. It can be a copy of:
- your current council tax bill
- your tenancy agreement
- your current TV Licence
- your driving licence
- a utility bill or bank statement dated within the last 12 months
- a rent statement dated within the last 12 months
- a benefit award letter dated within the last 12 months
Proof of benefits you get
We need a copy of all pages of your benefit statements or award letters.
For PIP we need all pages of your statement including the end date and points given in each category.
Proof of disability or medical condition
This can include copies of:
- diagnosis letters
- medical assessment reports, for example, an audiologist's report
- a letter from your GP describing how the diagnosis affects you
- hospital consultant letter
- Certificate of Visual Impairment (CVI) - if you're blind or partially sighted
We'll also ask you about:
- how your disability or condition affects you
- medication you take
- surgery or treatment you've had
The eligibility criteria are complex. We'll contact you if we need more information.
If we ask you to fill in a medical fitness to drive form, we'll send this to you. It must be signed by a specialist and stamped by their practice or organisation.
