End of life planning

If the person you care for is nearing the end of their life, there are some decisions you may need to make together about their care. 

This is to help understand their wishes. It also helps take the pressure of making those decisions off of you.  

You may want to talk to find out: 

  • where they want to be cared for when they’re dying  
  • who they want to be with 
  • what makes them feel safe and comfortable  
  • what treatment they’d like or if they’d like to refuse specific treatment 
  • who they’d like to appoint to make decisions for them if they’re unable to
